Hugh Hornby was born on 15 June 1890 and joined the Royal Navy as a Cadet in 1906. After time in surface ships, he started submarine training as a Lieutenant in August 1912.

After time in Depot Ships, he was appointed to HMS D6 as First Lieutenant in December 1914. He took command of HMS A5 in August 1915 and C26 in April 1916.

Hugh Hornby died on 1 May 1916 from injuries sustained while attempting to dive off the deck of the Depot Ship.
