JASTRZAB started life as the US submarine S-25. She was decommissioned and transferred to the Royal Navy on 4 November 1941.

Because of a shortage of trained submarine crews within the Royal Navy, the boat was lent to the Polish Navy and named JASTRZAB (Hawk).

JASTRZAB (Lt Cdr Boleslaw Romanowski) was mistakenly sunk by the Norwegian destroyer ST ALBANS and the British minesweeper HMS SEAGULL in the Norwegian Sea in position 71 30’N, 12 32’E. Five men died.
