HMS P222 (Lt Cdr A J Mackenzie) sailed from Gibraltar on 30 November 1942 for a patrol area off Naples. Nothing was heard from her after 7 December and she failed to arrive at Algiers on 21 December, as expected,

The Italian torpedo boat FORTUNALE reported carrying out an attack on 12 December 1942 in position 40 29’N, 14 20’E – southeast of Isola di Capri and this is thought to be the most likely cause of the submarine loss.

There were no survivors.
