Whilst joining a Convoy off Bizerta to return to Malta, HMS UNISON was fired on by an American tanker which mistook the submarine for a German U-Boat. Of the bridge crew the Commanding Officer, Lieutenant Daniell, lost two toes, Leading Signalman James Halliday was injured as he fell from the bridge into the control room and Petty Officer Coxswain George Day injured his legs. The Officer of the Watch, Lieutenant King, was severely wounded and died later of his wounds. He was buried at sea.
